FORMAN, Peter  Age 77, of Swartz Creek, died Thursday, January 15, 2004. Services will be held 11 AM Monday, January 19, 2004 at Sharp Funeral Homes, Miller Road Chapel, 8138 Miller Rd., Swartz Creek with Deacon Gerald OToole officiating. Visitation will be held 2-4 and 6-9 PM Sunday at the funeral home. A Masonic Memorial Service will be held 6:30 PM Sunday evening at the funeral home under the auspices of Byron F&AM Lodge #80. In lieu of flowers, those desiring may make memorial contributions to Byron F&AM Lodge #80. Mr. Forman was born July 20, 1926 in Cleveland, OH, the son of Mike and Esther (Rustiak) Juhas. He married Claudia M. Shook on December 20, 1974 and she preceded him in death on December 20, 1995. Mr. Forman was a Die Maker with General Motors Metal Fabrication for 45+ years retiring on January 1, 1991. He had resided in Swartz Creek since 1973. Mr. Forman was a veteran of the U.S. Army serving during World War II in 310th Infantry Battalion 78th Division. He was a member of the Masons 32nd degree, former member of the Shriners, a member of Byron Lodge F&AM #80. Mr. Forman played bagpipes in the Highlanders Band in Flint. Surviving are: two sons, Kyle P. Forman of Colorado Springs, CO, Paul H. Forman of Swartz Creek; grandson, Brian Forman of Arlington, VA; step-daughter, Ann and husband, Greg Semple of Swartz Creek; step-grandsons, Brandon and Jason Semple of Swartz Creek; sister, Helen Telemahos of Owosso; brother, Michael Forman of Durand; sister-in-law, Mary Forman of Durand; niece, Helen and husband, Don Pratt of Lennon; aunt, Ann Figel; cousin, Walter and wife, Dee Figel, all of Castle Rock, CO.
